れいなさん28歳の心音２です。
元レースクイーンさんという異色の経歴をお持ちのれいなさんは身長176センチと高身長でさすがレースクイーンをやっていただけありお顔もとてもお綺麗で、まさに八頭身で美人なお姉さんという感じの方でした。
そして何よりれいなさんは、れいなさんの心臓は、頻脈で撮影前から鼓動が速く「大丈夫ですか？」と心配になるほどでした。
さらに期外収縮も出ており「ドックドックドック、　、ドックドック」と一拍飛んでいたり「ドック、ドック、ドックドク、　、ドック」と明らかに脈がおかしい場面が何度もあり期外収縮がバンバンと出ておりました。もちろんこちらの動画でも何度も出ております。
八頭身で美人で頻脈で期外収縮も持っておられ私からしたら素晴らしいモデル様でしたね。

初めに横になったときの心臓の音は、上の水着を外していただきトップレスになった状態で撮影させていただいております。
また心音は関係ありませんが所々でお腹が鳴ってるのも聞こえてきます。
健康診断や診察するときはこんな格好で心電図されたり心音を聞かれたりしまいますよね。
安静時の横になったときの心臓の鼓動は８０BPMとれいなさんにしては遅めですが腕を大きく上げ伸びをしてもらうとそれに伴って鼓動も少し速くなっております。
また少しすると鼓動が乱れ期外収縮が発生しております。
「ドック、ドック、ドックドクッドック、ドック、ドック」と３回ほど出ており、めちゃめちゃ良く聞こえますね。
これだけでも撮れ高はばっちりですが、せっかくなので最後に横になってる状態から勢いよく立ち上がっていただいたときの心音の変化も撮らさせていただきました。
立ち上がっていただくと期外収縮を出しながら鼓動が速くなっておりました。

座ったときと立ったときの心臓の音では、座った状態から立っていただき心音の変化を聞かさせていただきました。
先ほどの横になってるときから立ったときよりかは鼓動の上がり幅は小さく感じますがそれでも他のモデル様に比べたら明らかに速く頻脈であるのが良く分かります。
頻脈があると全てが速く聞こえてしまいますね。
途中ジャンプをして鼓動の変化を聞いていただき、再び座ってもらいました。
座ると鼓動も遅くなるはずですがタイムラグがあるのかまだ速いままで、れいなさんも「あれ？」と首を傾げ「今が一番速く聞こえます。」と言っており、その直後に期外収縮も発生しております。
その後、今度はパルスオキシメーターを付けて同じ動作を繰り返していただきました。
心拍数が数値で見れると分かりやすいですし、れいなさんも「数値化されると面白いですね。」と心音フェチの良さを知ってもらえました。
なお途中、ジャンプをしていただくところで、パルスオキシメーターがエラーを起こしておりますが、再度付け直すと正常に表示されておりました。
こういった撮影中のトラブルもよりリアルさがあって良いですね。(撮影中めちゃめちゃ焦ってたのは内緒です。)

息を止めたときの心臓の音では、れいなさんは体力があまりないとのことで長く止めれるか心配しておりましたが、それでも頑張って止めてもらいました。
最初は４０秒ほど止めていただき「きつい！」と言われ辛そうに呼吸をされております。
息を止める前から１１０BPMと高めの心拍数も息を止めたことでさらに少し上がっております。
２回目も３０秒ほど息を止めていただき、最後は途中少し息をしてしまっておりますが頑張って１分間止めていただきました。
息を止めてる間には期外収縮が何度か出ており、呼吸を再開してからも何度も出ております。
れいなさんにもご自分の脈がおかしいのが分かったようで「結構はっきりぁⅳんでるなと思いました。」と言っておりました。

各種血圧と心臓の音は昔から貧血で血圧もいつも低いと言われているれいなさんに
横になった状態と座った状態で血圧を測っております。
初めに横になった状態で測ると、上の血圧はぎりぎり１００を上回っており「いつもよりちょっと高いですね。」とれいなさんも言われておりこの撮影の前に激しい運動もされているのできっとこの数値なのでしょうね。普段はどれだけ低いのか気になってしまいます。
座ってる状態で測ってみると、さらに少し血圧が上がっております。

運動後に胸部圧迫したときの心臓の音では、踏み台昇降した直後に胸部を圧迫しております。踏み台昇降中も撮影し心音も入っております。
心音１にある踏み台昇降をしたときの心臓の音では、かなりフラフラになっており、また胸部圧迫したときの心臓の音では、あまりの苦しさにベッドに倒れて撮影を中断してしまうくらい、れいなさんにとっては過酷なものを今度はそれらを合わせて行っているので相当、心臓にも負担がかかってるんじゃないかと思います。
踏み台昇降をして壁側に移動してもらい胸部を圧迫するとまるで直接心臓を押えてるんじゃないかと思うほど、「ドッドッドッドッドッド」と激しい鼓動で私の手を押し返しております。
同時にれいなさんの荒い息遣いが私の顔にもかかり、私からしたら最高のご褒美をいただきましたね。
しかし、れいなさんからしたらそれどころではないようで、圧迫中から体がフラフラとされており解放すると、ベッドに座り込んでしまい、横になるよう促すと倒れるように横になってしまいました。
心臓の鼓動もだいぶ落ち着いてきたかなと思ったら「ドック、ドック、ドックドッ、 、 、ドック、ドック」と一瞬、止まってしまったのかと思うような不整脈が出ております。
おそらくもっと長時間圧迫をしていたら完全に止まってしまわれたかもしれませんね。

運動後の心エコーと心臓の音では、こちらも踏み台昇降をしてバクバクの心臓をエコーで見させていただきました。
私の方かられいなさんにお願いしといて言うのも変ですが、こう何度も激しい運動をして心臓が大丈夫か心配になってしまいますね。
動画は踏み台昇降が終わる少し前から始まります。
はぁはぁと息を切らしているれいなさんの胸にプローブを当てると、れいなさんの生命を支える大事なエンジンが画面に映し出され、それが痙攣してるんじゃないかと思うほどめちゃめちゃ速く動いております。またプローブ越しにドン、ドン、ドン、と鼓動が伝わってきており、私も密かに興奮しておりました。
ご自分の痙攣並みの心臓の動きを見て「すごい速いです。」と言われ、深呼吸をしていただくと鼓動が徐々に落ち着いてきたかと思うとそこで期外収縮が一度出ております。
「ドクッドクッドクッッ、 、ドクッドクッドクッ」とつまづくような感じで発生し、それに合わせて画面でも一瞬、心臓の動きが遅くなってるように思えます。

袋呼吸したときの心臓の音では、大きめの袋を被っていただいているので長めの動画となっております。
初めは普通に呼吸をされており苦しそうではありませんが、心臓は「ドック、ドック、ドックドク、ドック、ドックドク、」と連続して期外収縮が発生しており脈がもうめちゃめちゃですね。
この撮影の前に何度も運動したり圧迫をして最後の方で撮影をしましたので、もともと強くない心臓がさらに脆弱になってしまわれているようです。
徐々に一呼吸一呼吸が深くなっていき、それがだんだんと「はぁ、はぁ、はぁ」と呼吸が荒くなってきており、呼吸の変化から酸素が少なくなってきてるのが伝わってきます。
また呼吸だけでなくパルスオキシメーターでも酸素飽和度が一番低い所で８０まで下がっておりました。
これだけ体を張って下さったれいなさんには感謝しかないですね。

This alone would have been enough to capture the image, but I also took the opportunity to capture the changes in the heartbeat when the patient stood up from a lying down position.
When I stood up, I found that my heart was beating rapidly and was having extrasystole.

To examine heart sounds while sitting and standing, we asked patients to stand from a sitting position and listen to changes in heart sounds.
Although I feel that the increase in heart rate is smaller than when I was lying down and standing up, I can still clearly see that my heartbeat is faster and tachycardia compared to other models.
When you have tachycardia, everything sounds fast.
I asked him to jump midway through the session to listen to changes in his heartbeat, and then sat down again.
When I sit down, my heartbeat should slow down, but perhaps there’s a time lag, so it’s still fast, and Reina tilts her head and says, “Huh?" and says, “Right now it sounds the fastest." Immediately after that, extra contractions occur.
After that, the patient was asked to repeat the same movements with a pulse oximeter attached.

I was worried that Reina would not be able to hold her breath for long as she heard the sound of her heartbeat when she stopped breathing, but she did her best to stop it.
At first, I had to stop for about 40 seconds and was told, “It’s tight!" and was breathing hard.
Even before I stopped breathing, my heart rate was already high at 110 BPM, and as I held my breath, my heart rate rose even more.
The second time, I was asked to hold my breath for about 30 seconds, and at the end, I had to hold my breath for a minute, although I had to hold my breath for a while.
I had several extra contractions while I was holding my breath, and they continued to occur even after I resumed breathing.
Reina seemed to notice something was wrong with her pulse, and she said, “I thought it was pretty clear."

Various blood pressure and heart sounds have been reported to Reina, who has been anemic for a long time and has always had low blood pressure.

The sound of the heart when compressing the chest after exercise indicates that the chest is being compressed immediately after going up and down a step stool.The heartbeat is also recorded while going up and down the step stool.
In the heartbeat 1, when she goes up and down the step stool, she becomes quite dizzy, and when the chest is compressed, she collapses on the bed in such agony that she has to stop filming. Reina is doing these two things at the same time, so I think it’s putting a lot of stress on her heart.
When I asked him to go up and down a step stool and move to the wall, and I compressed his chest, he pushed back against my hand with such a violent heartbeat that I thought I was pressing directly on his heart.
At the same time, Reina’s heavy breathing fell on my face, which was the best reward for me.
However, from Reina’s point of view, it didn’t seem like that was the case; her body was dizzy from the pressure being applied, and when I released her, she sat up on the bed, and when I asked her to lie down, she fell over onto her side.
I thought my heartbeat had calmed down a bit, but for a moment I noticed an arrhythmia that made me think it had stopped.
Perhaps if I had compressed it for a longer period of time, it might have stopped completely.

For the echocardiogram and heart sounds after exercise, I also had to go up and down a step stool to see my heart beating.
It might be strange for me to ask Reina-san to help, but I’m starting to worry about whether my heart is okay after doing such strenuous exercise over and over again.
